Patient Rights and Responsibilities

As a patient, you have rights and responsibilities, which include the following:

  1. You have the right to receive treatment without regard to race, color, national origin, religion, sex, gender identity, sexual orientation, disability or ability to pay.

  2. You will be treated with respect and sensitivity for the psychosocial, spiritual and cultural values that may affect your response to care.

  3. You have the right to participate in the development and implementation of your plan of care and to obtain relevant, understandable and current information regarding diagnosis, treatment and prognosis. This also applies if you speak languages other than English or use communication aids for limited hearing and sight.

  4. You or your representative has the right to make informed decisions regarding care. This includes being informed of your health status, a description of a procedure or treatment, alternatives to the choice of treatment and risks involved in the course of treatment or non-treatment.

  5. You have the right to designate who may accompany you on your visit or who may visit you during your stay, regardless of whether this is a family member, a spouse, a domestic partner (including a same-sex domestic partner) or any other type of visitor.

  6. You have the right to have a family member or representative of your choice and your own physician notified promptly of your admission to the hospital.

  7. You have the right to express advance directives and to have hospital staff and practitioners follow these directives in accordance with state law.

  8. You have the right to the management of your pain. This includes the timely assessment, intervention and monitoring of the effectiveness of treatment by hospital staff and practitioners.

  9. You have the right to personal privacy.

  10. You have the right to receive care in a clean, safe and secure environment.

  11. You have the right to courteous treatment and quality medical care in recognition of human dignity. You are assured of freedom from abuse, neglect and harassment by staff.

  12. You have the right to privacy of your clinical record. The hospital ensures the safeguarding of information, including paper records, videos and computer-stored information. You can make a specific request to release your information.

  13. You have the right to access your clinical and financial records within a reasonable time as allowed by law.

  14. You have the right to be free from any form of restraints that are used as a means of coercion, discipline, convenience or retaliation by the staff. Restraints can only be used when deemed medically necessary.

  15. You have the right to continuity of care, transfer and consultation with other medical specialties.

  16. You have the right to voice or file a grievance.

  17. You have the right to request and have access to protective and advocacy services.
