Hearts of Hope (CHF Program)

The Congestive Heart Failure Hearts of Hope program is designed to help participants with congestive heart disease take control and make changes to feel better and live an active, healthier life. The program takes two weeks to complete four sessions, which meet on consecutive Tuesdays and Thursdays. Each session lasts two hours and includes exercise, education, counseling and learning ways to live a healthier life. A registered nurse, exercise physiologist, pharmacist and registered dietitian staff the program. Physician referral is required. Benefits of the program include:

  • Understanding what you can do to help yourself feel better and live as active and healthy as possible
  • Learning to monitor your condition and knowing when and how to seek help
  • Beginning or refining your exercise program and learning activity guidelines to improve endurance and strength
  • Assessing your current diet and making realistic changes
  • Meeting with a pharmacist to review your current medications and learn about the actions, side effects and potential interactions as well as developing solutions to difficult drug schedules and regimens

The center is located in the Pardee Rehab & Wellness Center on Thompson Street in the East Ridge Shopping Center.  Our staff will communicate your participation and progress to your physician. To learn more about the CHF program, ask your physician or call 698-4694.
